WORKING ADULT PROGRAM

This Program is designed for working low income adults who live in Barren County. If qualified you will receive basic health care for a $10.00 co-payment through T.J. Samson Family Medicine Center. Other participating facilities provide basic eye, dental and consultation with specialist, basic physical therapy, psychological counseling. All of these services are offered on a volunteer basis.

If approved each qualified individual will receive a CMC I.D. Card. This card is good for 3 months. When medical care is needed you must call the T.J. Samson Family Medicine Center at 270-651-4797 to schedule an appointment.

When eye care or emergency dental is needed you must call the Community Medical Office at 270-659-0229 for a referral to a participating facility.

Some services may not be covered through Community Medical Care Program. In this event the Client/Patient will be advised and may be responsible for services. This card will not cover Emergency Services.
